Have you been meaning to complete an Advance Healthcare Directive, but for one reason or another, just have not done so yet?
The free program will provide information about Advance Healthcare Directives. How they work, what choices you can make about the type and degree of healthcare treatment you would want based on conditions one could be in. Who you want designated to make healthcare treatment decisions for you if you are unable to do so. In addition, attendees can complete an Advance Healthcare Directive at the program.
Date: Wednesday, April 13, 2016
Time: 10:30 A.M, and at 2:30 P.M.
Location: John C. Fremont Hospital, 5189 Hospital Road, Mariposa, in the Board Room of the Administration Building, in back of the hospital.
